PHOTO RELEASE AGREEMENT
In consideration of my engagement as a model, and for other good and valuable consideration herein acknowledged as received, I

_________***************____________________ (the “Magazine Publisher”), his/her heirs, legal representatives and assigns, those for whom the Magazine Publisher is acting, and those acting with his/her authority and permission, the right to the photograph of me on the date and at the location listed below for any use, reuse, and/or publishing and republishing of in any media and for any uses, including, but not by way of limitation, illustration, promotion, advertising and trade, and to use my name (or any fictional name) in connection therewith.
I hereby agree to release, defend, and hold harmless the Magazine Publisher, his/her heirs, legal representatives and assigns, those for whom the Magazine Publisher is acting, and those acting with his/her authority and permission, including but not limited to, any firm publishing, distributing or otherwise using the finished product in whole or in part, whether on paper or via electronic media, including but not limited to any misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction or production of the finished product, its publication or distribution.
I am 18 years of age or older and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release. This agreement shall be binding upon me and my heirs, legal representatives and assigns.
